For Families · Superbills & Reimbursement

Your Superbill. Your Insurance. Less Guesswork.

If you've paid out of pocket for therapy, your insurance plan may allow you to request reimbursement.

We can help with the paperwork and submission process so you're not left figuring it out alone.

The process

1

Gather the Documentation

We'll help coordinate the information needed for a superbill with your treating provider.

2

Submit the Claim

With your authorization, we can submit eligible superbills through supported insurance submission processes.

3

Follow Up

We can help track claim status, respond to administrative requests, and explain the information returned by the insurer.

Good to Know

  • Your treating provider must verify all clinical information, diagnosis codes, service dates, procedure codes, and provider credentials.
  • We do not invent or alter clinical information.
  • We do not guarantee reimbursement. Some plans may not offer out-of-network benefits.
  • Some payers may require member-direct submission.
  • Claims are submitted through supported processes only; we name the submission service when it is actually in use.

What do you need?

Please do not include insurance member IDs, card images or medical documents in this form. When we help with a superbill or claim, your treating provider verifies all clinical information, and submitted details are handled with the appropriate authorization.
